VVE Community Session: Advanced Care Planning

What matters most to you when you think about your health changing?
Advance Care Planning is the process of reflecting on your values, goals, and healthcare preferences for both the present and future. It allows you to communicate the care you would like to receive and ensure your wishes are respected. In this session, Cheryl, an ACP facilitator and nurse specialist, will provide guidance, resources, and information to support you in this process.
What are the benefits advance care planning? Advance care planning encourages conversations about what is important to you and your family and can help you feel more in control, reduce anxiety and give you peace as you face the future.
