East Health Trust was established as a Primary Health Organisation (PHO) on 1st July 2003 and provides primary healthcare services in the Howick, Botany, Pakuranga, Beachlands, Clevedon, Flatbush, Maraetai, Kawakawa Bay and Pukekohe areas.

Social Connection at Howick Selwyn Centre.
The weekly get-together offering friendship, aroha and inclusion. Selwyn Centre provides a welcoming, easy-to-access community space for older people to meet, socialise and enjoy activities with other seniors in their area. Each session includes an organised programme of social activities, gentle exercise (chair-based and/or standing exercises), as well as morning tea – see below full details.

With a vision of providing a centre of excellence that enhances lives, Pakuranga Medical Centre is dedicated to helping people with any and all health issues. A member of the East Health Trust, they offer subsidised visits to enrolled patients and a wide range of programmes for patients with higher needs.
Driving Miss Daisy is ideal if you don’t drive or no longer drive at night or greater distances. It provdes transport and extra support to help you get out and about, including going to appointments, shopping, company on outings and help getting in and out of the car. Special needs are catered for including assistance with a walker or wheelchair.
Freedom Drivers provide a caring, friendly and reliable alternative to taxis. If you need extra help, they’ll assist you from your door to the car, accompany you to appointments or pick you up afterwards. They also provide companion assistance with shopping, picking up scripts, going out for a cuppa or scenic drive, and help with walkers or wheelchairs.
Senior citizens can travel for free on trains, selected bus and ferry services in Auckland, after 9am weekdays and all day weekends and public holidays with an AT HOP card loaded with a SuperGold concession.
The Total Mobility scheme helps those with serious mobility constraints access subsidised door-to-door transport services. If you’re eligible, you’ll receive a card providing a 50% discount (up to $40 in Auckland) on fares charged by contracted transport providers.

Giving Time is a nursing consultancy service that assists older citizens to access first class support. Founded aged care specialist, Kaye Davies, it’s designed to help you navigate potential changes in your health circumstances.
Every week, EAT makes a fresh batch of handmade meals using only good ingredients and great cooking! They’re delivered fresh, not frozen, so you can have a fresh meal that’s ready to ‘heat and eat’ for up to 10 days.
The Meals on Wheels service provides nutritious, home-delivered lunch meals to anyone who can’t reasonably manage to prepare meals for themselves.
